GILEAD - Case Manager

Clinical Case Manager-Residential Program (Adult) 12CM1 Gilead Community Services is seeking an attentive and recovery-oriented Clinical Case Manager to join our multi-disciplinary team in our supervised apartment program for men. About the position: The Clinical Case Manager for Gilead's Middletown adult supervised apartment program works Mon 1p-9p, Tue & Wed pm-11m, Fri 1m-9m, EOW 3p-11p providing case management, advocacy, educational, and supportive services in a program serving men with major mental illness who require intensive supports to reside in a community setting. The individual in this role carries out all phases of clinical services including intake, engagement, assessment, treatment planning, case management, referral, and discharge. This role also requires the timely completion of required documentation including assessments and other treatment planning. Responsibilities: Providing clinical knowledge regarding severe and prolonged mental health and substance abuse disorders through training and education Working with the treatment team to manage clients' mental health and assessing for risks, implementing risk management plans as necessary Providing recovery-oriented clinical case-management services with an emphasis on dignity, respect, and inclusion of family and other natural supports Providing support, skills training and education in areas including daily living skills, nutrition, financial management and symptom management Monitoring positive outcome measures and ensuring program compliance through accurate and timely data reporting Completing assessments, treatment plans, and other documentation electronically and on paper in a timely manner Acting with compassion, integrity, and ethics Gilead employees are held to standards and regulations set by the Department of Mental Health and Addiction Services (DMHAS) and other regulatory bodies.
