Assesses, plans, implements, evaluates total patient care and coordinates patient/family teaching. Assures compliance with therapy standards, federal, state, and local government regulations, and company clinical policies. Provides goal directed care leading to specific outcomes. May be on call for after-hours work. Coordinates and directs the home care patient's care based on individual patient needs. The value you'll bring to the team: Performs an evaluation, assessing function using a method which objectively measures activities of daily living such as, but not limited to, eating, swallowing, bathing, dressing, toileting, walking, climbing stairs, using assistive devices and mental and cognitive factors, documenting the measurement results in the clinical record. Administers treatments as discussed in the plan of care involving application of physical agents, and manual techniques to improve and maintain function. Instructs, motivates, and assists the patient to perform various functional physical activities, activities in daily living, self- care activities, and exercises. Measures and monitors effects of treatment at various stages and confers with occupational therapist to adjust treatments to achieve maximum benefit and outcome. Assures that patient treatment outcome is directed towards development and restoration of functional skills and returning to or maintaining maximum potential. The expertise and experiences you'll need to succeed:Minimum qualifications : Minimum of six-months relevant occupational therapy experience BLS Certification Active Driver's License Occupational Therapy license Prefer recent, relevant experience in a Medicare-certified home health agency as an Occupational Therapist Prefer a Master's or Doctorate degree in Occupational Therapy For prompt and confidential consideration please click the link to submit your resume: Click here to apply online
