Location(s): Costa Mesa, CA, Grand Prairie, TX, Tampa, FL, Miami, FL, Overland Park, KS, St. Louis, MO, Chicago, IL, Mendota Heights, MN, Cincinnati, OH, Iselin, NJ, Nashville, TN, and Wilmington, DE Work Hours: Monday - Friday plus 2 weekends a month Eastern Time: 8:00 AM - 7:00 PM Other Time Zones: 7:00 AM - 6:00 PM, 8:00 AM - 7:00 PM, or 9:00 AM - 8:00 PM The Carebridge Transitions of Care Nurse plays a crucial role in delivering patient education, managing chronic diseases, and providing health coaching tailored to members across various health improvement programs. Your Impact: Main Responsibilities: Conduct comprehensive assessments to understand the individual member's knowledge, skills, and behavioral needs. Coordinate specific health coaching plans to meet the goals identified in assessments. Engage with providers and healthcare professionals to ensure efficient implementation of health coaching plans. Provide education on clinical needs and refer members to behavioral health professionals as necessary. Utilize motivational interviewing techniques to drive health behavior changes. Monitor and assess the effectiveness of health interventions and adjust plans as necessary. Assist members in connecting with facilities, community resources, and appropriate providers. Direct members to catastrophic case management as needed. Minimum Qualifications: Requires an Associate's degree in nursing and a minimum of 2 years of specific clinical or home health/discharge planning experience, or a suitable combination of education and experience. A current unrestricted RN license in applicable states is required. Preferred Skills and Experience: A compact RN license is highly preferred. A Bachelor's degree in nursing is preferred. Experience in case management is preferred. Bilingual in Spanish or Mandarin is highly preferred. Experience with chronic disease management is highly preferred. Background in home health settings is preferred. For candidates working in person or virtually in the below locations, the salary range for this position is $65,600 to $113,160.
