Assign homecare aides based on clients' needs and coordinate the Plan of Care effectively. Provide coaching and support to caregivers to ensure high-quality client care, addressing performance concerns as needed. Communicate proactively with clients, home care aides, referral sources, and management regarding scheduling and other changes. Conduct in-home visits to assess client needs and maintain relationships, while documenting insights and updates. Prepare and submit routine departmental reports, maintaining accurate client condition documentation. Address client complaints constructively and escalate significant issues for resolution. Educate and guide caregivers on updates to client Plans of Care. Foster positive relationships with clients, homecare aides, and referral sources. Ensure compliance with local, state, and federal regulations, as well as company policies. Perform other duties as assigned, with the flexibility to adapt to changing needs. If applicable, comply with state regulations regarding training and documentation for Developmental Disabilities. Qualifications: What You Bring: Strong organizational skills with the ability to multitask effectively. Capability to provide constructive feedback and work towards personal and professional growth. Excellent problem-solving abilities, with a professional approach to conflict resolution. Compassionate demeanor with a commitment to quality service. Basic computer literacy and good typing skills. Outstanding customer service skills. Education and Experience: Must be at least 18 years of age. High school diploma or equivalent preferred; additional education or certification may be required. Current PPD or Chest X-ray (if applicable). Preferred experience in Medicaid, Waiver, or Home Healthcare. Adherence to state or local regulations as needed. Management Authority: Conduct performance reviews and provide training to fellow associates. Direct the work of other team members. Physical and Travel Requirements: Ability to transport or position up to 50 pounds and navigate client homes, including stairs. Effective communication skills to exchange information clearly.
