Job Description

Job Type Full-time Description IPS Case Manager will provide case management services to IPS youth and their families. The IPS Case Manager will be responsible for conducting assessments, monitoring safety in home and program, monitoring school progress & attendance and providing referral and linkages. He/She will provide on-site programming including, but not limited to, academic workshops, tutoring, recreation, field trips, etc. IPS Case Manager will report to the site IPS Counselor and IPS Supervisor. Requirements Provide Case Management to youth and families of the IPS program, including monitoring youth in home & on site, provide referral & linkage and participate in crisis intervention Conduct mandatory home visits when required, with the assigned Counselor Conduct and or participate in assessments of youth with parents/guardian input. This includes intake, Service Plans, Discharge Plans etc. Collect information about the family through use of interview, case history, and observational techniques Link parents to support services related to Housing or Social Services Participate in monthly case review meetings with the team and DHS liaison to discuss youth progress Document all services provided for each assigned case including, but not limited to, assessments, progress notes, service plans,monthly and narrative reports both in databases and hard file Make court appearances with all youth on assigned caseload Maintain excellent relations with the assigned Counselor and the team Attend all required meetings and trainings Adhere to IPS and DHS standards, protocol and expectations Participate in transporting clients to and from program, as needed Other duties as assigned Qualifications and Experience Requirements: Bachelor's Degree in relevant field (social work, behavioral health counseling, criminal justice, or related education) Master's degree preferred. At least three years of direct service experience working with urban youth and/or families, e.g., individual and/group counseling, youth work Strong interpersonal & communication skills Demonstrated ability to work as part of a team and in collaborative environment Working knowledge of Microsoft Office applications and maintaining database Excellent organizational skills, ability to effectively handle documentation and paperwork Bilingual/Bicultural in Spanish and English Strong communication skills-written and verbal Strong interpersonal skills and demonstrated ability to interact with individuals of different racial, cultural, educational, and economic backgrounds Valid driver's license and car or strong knowledge of local Septa routes Familiarity with the Norris Square Community and its service providers Salary Description $43,888.00
