This position may be filled by a Physician Assistant-Certified or an Advance Care Nurse Practitioner (ARNP). Under the supervision of the Health Services Managers/Facility Medical Director, this position contributes to the agency's mission to improve public safety by delivering quality health care to the facility patient population in a timely and professional manner, promotes healthy life-styles, and provides patient health education. This includes assessing, diagnosing, and treating the multitude of medical issues encountered. This position monitors, manages, coordinates and delivers health care and counsels, consults, teaches, advises and provides health care information to the patient that improves the quality and continuity of health services. This position monitors all levels of health care services delivered to the incarcerated patient population and assists the Health Services Managers and the Facility Medical Director in monitoring expenditures, planning and organizing health care services, facilities and other programs. The Washington Corrections Center for Women (WCCW) is a 62.6-acre facility located approximately two miles from Gig Harbor. The site includes living units, administration buildings, a health clinic, education buildings, a gymnasium, a chapel, industries buildings, food service buildings and support facilities. Correctional Health is a rewarding career path at DOC, in which health care professionals treat a wide array of cases for patients residing in a state correctional facility. Health Services is responsible for basic primary care for incarcerated individuals. All disciplines within all of the 12 correctional facilities work closely together to reach a common goal, providing the best, most efficient, and cost-effective health care possible. Emphasis is placed on early identification of health concerns, care for acute and chronic health problems, and preventive care. Duties Duties include: Provide direct patient services Diagnose health related problems and determine outcome. Ensure continuity of care. Deliver health care, defining time frames and pathways for delivery of services, and determining priorities and sequences of care to be provided. Implement plan of care by providing direct patient care, directing others about the urgency, timeframes and methods to deliver health care on and off site; determining when need for care or services becomes more urgent, need clarification, or another type or level of intervention; and taking action as necessary to provide orders and accomplish prescribed interventions in a timely and clinically responsive manner. Analyze and evaluate the structure, process, and timeframes for service delivery, the expected effects of treatment or intervention, the outcomes to be achieved, revision of the plan of care to prevent complications and intervene to address new or emerging problems or to improve care and service delivery. Perform minor surgical procedures and suture of simple lacerations. Order and interpret appropriate diagnostic laboratory tests, ECGs and X-rays. Prescribe medications including controlled substances and buprenorphine as required. Refer complicated and complex cases to staff physicians and outside medical consultants and facilities when necessary. Admit and care for infirmary patients when needed. Provide urgent and emergent medical care both in the infirmary and in living units or other facility areas. When applicable, manage a population of primary care patients in general population or in an intensive management/segregation unit. Update PULHES codes to reflect changes in the patient's condition or circumstances. Complete Screenings/Physical Examinations Complete initial screening and perform initial physical examination. Initiate PULHES coding and other health classification requirements. Provide effective communication and documentation Effectively communicate verbally, in writing and via computer or other electronic media with offenders, their families, other health care personnel, custody and other institutional personnel. Legibly document subjective and objective clinical findings, as well as the actions taken to deliver all aspects of health care in the health record computerized database and on other DOC approved forms. All documentation will be in accordance with the Health Care Guidelines. Protect the confidentiality of health records as mandated by law, professional standards, DOC Policy directives, facility Operational Memoranda, and Health Care Unit procedures. Treat any and all health records or health information as confidential and privileged. Maintain accurate, legible, and complete records of patient care including chart documentation using appropriate format of all patient requests for medical attention. Assist in writing, evaluating and revising treatment administered, and patient education provided. Assist in writing, evaluating and revising nursing care plans and interdisciplinary treatments plans, and complete other entries to support the orientated health care record. Communicate and assist in the coordination of pertinent information to health care providers for patients scheduled for community-based care. Complete unusual incident reports as directed, collect data, investigate and complete reports necessary to the ongoing operation of Health Services and the institution. Respond to verbal and written requests from Incarcerated Individuals and institution staff as directed, with information or advice consistent with statues and procedures on health care record confidentiality. Read and respond to email daily during work week. Actively participate in clinic/facility-wide Coordinated Quality Improvement Program (CQIP) meetings, projects and programs. Attend facility CQIP/QMP meetings and report back to nursing and support staff to keep nursing staff engaged in improving patient care at every level, on every shift, and at every bedside. Serve as the lead for at least one local quality improvement project per year and engage staff in the process at some level to ensure all employees have an awareness and understanding of the principles of quality improvement and the goals for the organization. Participate/represent facility on state-wide quality improvement initiatives; share the WCCW processes and learn from other facilities. Participate in Health Services Quality Improvement activities. Practice in accordance with DOC Policy and Offender Health Plan. Participate in formal clinical oversight according to facility and DOC Health Services Clinical Oversight Structure. Delegate and assign patient responsibility through appropriate communication to the nurses and assistive nursing personnel. Evaluate and consider the needs, condition and stability of the patient, potential harm, complexity of task and predictability of outcome to determine appropriate assignment. Teach, instruct, monitor, supervise and evaluate the implementation of patient care tasks. Collaborate with management to facilitate the implementation for processes that affect nursing staff, custody, support staff, and other departments that impact patient care. Consult with custody and other staff concerning health and safety of offenders. Provide medical health and safety advice with regards to offenders to custody staff when necessary and appropriate in accordance with DOC policy and compliant with HIPAA. Provide medical input to respond to routine and emergency grievances. Respond to incarcerated individual's KITES. Other duties as assigned. QualificationsRequired Qualifications: Certification as a Physician Assistant by the National Commission on Certification of Physician's Assistants. OR Must possess a valid Washington State license to practice as a Registered Nurse AND possession of a current license designated as an Advanced Registered Nurse Practitioner in the state of Washington. AND Must hold unrestricted license to practice as a midlevel practitioner and prescribe scheduled drugs. Licensure (PA-C, RN, ARNP) must be valid and continuously unrestricted. Preferred/Desired Qualifications: Experience working as a mid-level health care provider at a correctional facility. Maintain a current DATA 2000 waiver/Buprenorphine waiver with a Drug Enforcement Administration (DEA) identification number. One year of working experience as an ARNP or PA.
