Nurse - Residential Services Program Position Summary The Nurse for the Residential Services Program plays a crucial role in ensuring that all healthcare-related policies and practices at the group home adhere to the highest standards of care and regulatory compliance. This position involves delivering direct nursing services, offering staff support through training and guidance, and advocating for the well-being of residents. Minimum Qualifications Current LPN or RN licensure in the State of Ohio At least one (1) year of nursing experience Training and/or experience with traumatized adolescents preferred; a willingness to complete trauma-informed care training is required Exceptional verbal and written communication skills Experience with adolescent populations and psychiatric medications preferred Ability to demonstrate flexibility and adaptability in a residential treatment setting Valid Ohio Driver's License with a good driving record Alignment with Central Ohio Youth for Christ (COYFC) and Gracehaven's Standards of Conduct and Statement of Faith Key Objectives Create a safe and supportive atmosphere for both staff and residents Assess and monitor the healthcare goals of each resident in line with their Individualized Service Plan (ISP) Evaluate compliance of medical-related policies and practices Identify healthcare training needs among staff Maintain regulatory compliance for nursing licensure Participate in events and meetings to foster teamwork Essential Duties and Responsibilities Ensure a Safe Environment Promote a culture of safety for residents and staff Perform responsibilities in accordance with agency policies and professional standards Manage healthcare-related supplies and inventory Assess, Document, and Implement Resident Healthcare Plans Establish systems to meet residents' physical healthcare needs through triage, direct care, and care coordination Conduct regular healthcare assessments; monitor medical components of each resident's ISP Communicate with the treatment team for any recommended adjustments Review ISPs and Behavior Intervention Plans (BIPs) to identify medical contraindications Maintain vigilance of resident health needs, including nutrition, medication education, and side effects Oversee medication management, storage, and documentation following physician orders Conduct quality assurance reviews for medical records Train staff regarding residents' medical needs and medication handling Assist in medical exams and evaluations Provide wound care and emergency responses as necessary Help create safety plans for residents at risk Ensure proper storage and disposal of medications Keep accurate medical records in both EMR systems and resident files Advocacy and Collaboration Work alongside the Director of Residential Services and other healthcare professionals Coordinate healthcare services with external providers and families Deliver wellness education to residents and families Advocate for residents' medical needs within interdisciplinary teams Take part in staffing and ISP reviews to provide healthcare input On-Call Responsibilities Join a 24/7 on-call rotation for medical escalations Respond to medical needs via phone, telehealth, or in person Collaborate with medical services for coverage Compliance and Professional Standards Maintain active nursing licensure and certifications Complete ongoing training for licensure and agency standards Ensure compliance with HIPAA regulations Organizational Participation Attend team and staff meetings regularly Engage in bi-weekly supervision sessions Support fundraising and organizational events Spiritual Integrity and Leadership As part of a faith-based organization, all staff are expected to embrace and model COYFC's Christian mission and values, which include: Living out COYFC's beliefs both at work and within the community Representing the mission and Statement of Faith of the organization Participating in Christian activities Seeking divine guidance through prayer for residents and the organization Sharing faith content with residents as desired Encouraging colleagues through Christian fellowship PI258d7a45a60f-26289-39514625
