What are we looking for? We are looking for a Part-time (20-hours) Nurse to provide health services to participants, in accordance with standard nursing practices. These procedures should be consistent with the mission and philosophy of St. John's Community Services and adhere to all Medicaid regulations. This includes the direct provision of health services to meet the individual needs of each consumer in a manner that is normal and respectful of their rights and dignity. Your overall essential responsibilities will be the following: The Registered Nurse works in collaboration with an interdisciplinary team to provide optimal health care and safety to the DE State Program consumers. The nursing process assesses, plans, implements, and evaluates care. The specific responsibilities of this position shall include, but are not limited to: Ensure health supervision of adult consumers in DE State programs, including: Provides oversight of physicians' prescribed medications during program hours as directed by physicians' orders, following standard nursing practices. Ensure completion of nursing assessments including record review, significant family member/provider interview, developmental history, and current physical status as assigned by the program's RN Perform routine screening for blood pressure, weight, and other specific concerns as noted by the consumer's program. Provide emergency care. Work with the Nursing & Medical Care Coordinator for health deficit identification. Maintenance of health records and immunization records, notifying significant family member/providers when updating is needed. Ensure completion of progress notes monthly with quarterly summaries as assigned. 2. Interpret all adults' special diets and nutritional requirements for the consumers and staff 3. Ensure preparation of health history and interpret special health needs as part of each participant's Individualized Support Plan or Care Plan program. 4. Provide health counseling, education, and training for staff, participants, and/or significant family members or caretakers, in collaboration with the Nursing & Medical Care Coordinator, with regard to: Interpretation of health information. Guidance and counseling regarding health behavior Recommendations regarding individual health conditions. 5. Perform other job-related duties as assigned. Secondary Functions: Participate in perspective interviews, Behavior Management Meetings, Interdisciplinary Team Meetings, ISP / ICP conferences with family member/provider, discharge, and referral processes. Assist in the development of standards for health care, control of communicable diseases, and emergency first aid. Maintain a professional knowledge base by attending continuing education courses and reading professional journals to increase knowledge base. Maintain confidentially of all health conditions, informing only staff with a need to know. Participate in related services meetings, general staff meetings, psychotropic drug review meetings, meetings with primary physicians, and other meetings as required or assigned by the Nursing & Medical Care Coordinator or Program Director. Participate in training workshops, including agency training requirements. Perform other job-related duties as assigned. Meet SJCS and the funding authority requirement of criminal background and record checks. Physical and environmental conditions include: Work environment involves everyday risks or discomforts that require normal safety precautions typical of such places as offices or meeting rooms, e.g., use of safe workplace practices with office equipment, and/or avoidance of trips and falls. Work environment housing vulnerable individuals that may be experiencing symptoms/effects of substance use and/or mental health disorders. Current license as a Registered Nurse in the District of Columbia is required. Must meet SJCS and funding jurisdiction current requirements for criminal background/records check. Valid health certificate to meet current funding jurisdiction requirements. Valid state-issued driver's license with current driving record in good standing required. Physical and Environmental Conditions: Work environment involves everyday risks or discomforts that require normal safety precautions typical of such places as offices, e.g., use of safe workplace practices with office equipment, and/or avoidance of trips and falls. Home environment involves everyday risks or conditions that require normal safety precautions such as stairs, rugs, appliances etc. and requires the avoidance of trips and falls. Additionally, medical work with consumers involves knowledge of medical protocols, exposure to human behavior, and other safety precautions.
