We provide care for adults residing in skilled nursing and assisted living facilities, as well as private residences, through a network of healthcare providers including physicians, nurse practitioners, physician assistants, psychologists, licensed clinical social workers, and clinical and administrative support staff. The Clinical Support Coordinator plays a pivotal role in enhancing the performance of the clinical care team by supporting and facilitating clinical services with assigned providers. This role involves working in the field alongside assigned providers and may require travel to facilities up to 60 miles from your home. Hours: Monday to Friday - 8-hour daytime shift Benefits: Fuel card to assist with commutes Paid time off and holidays Health benefits (medical, dental, vision) Short & long-term disability options Employer contribution to 401k retirement plan Provider Support: Facilitate patient care by ensuring the clinical provider can focus on their responsibilities. Ensure each patient is seen as medically necessary according to clinical protocol. Serve as a liaison between providers, families, and staff to manage patient care effectively. Document encounters and time for Chronic Care Management (CCM) and Behavioral Health Integration (BHI) services accurately. If assigned to scheduling, maintain and update rosters daily. Assist in managing day-of changes to scheduled visits due to acute needs, hospitalizations, admissions, or discharges. Prepare clinical documentation daily for assigned providers. Perform medication reconciliations for scheduled patients at each encounter. Enter facility chart data and lab results into patient encounter notes. Assist with clinical paperwork and ensure timely completion. Host telehealth sessions as assigned. Facility Support: Share patient schedules with key facility staff as needed. Assist with triage needs under assigned provider’s direction. Ensure all necessary paperwork is routed properly. Support discharge planning as required. Conduct daily check-ins with facility staff regarding patient status. Patient Support: Maintain patient confidentiality at all times. Document appropriate encounter and time for CCM and BHI services. Check communications frequently to address patient needs. Inform patients and families of recent visits and defer to providers when necessary. Facilitate communication of care transitions to the CCM team. Qualifications/Education: High School Diploma or equivalency required. Valid driver's license and reliable transportation are essential. Expected to travel up to 1 hour from home address. Certification as a Clinician Support Specialist is a plus. Typing speed of 45 words per minute or greater. Medical scribing experience is preferred.
