Reports to: Director of Nursing JOB DESCRIPTION SUMMARY We are seeking a dedicated Physical Therapist to provide high-quality therapy services while adhering to all conditions outlined in the Personnel Services Agreement. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Deliver physical therapy services to patients following a physician's written plan of care, which includes: A. Assessing and evaluating therapeutic and rehabilitative status, participating in the overall care plan development. B. Analyzing muscle strength, mobility, gait, and range of motion to determine rehabilitation potential. C. Directing effective physical therapy treatments. D. Educating patients and families/caregivers on the use and maintenance of therapeutic devices. E. Identifying priority needs for therapy. F. Reporting patient responses to treatment and any changes in condition to the physician. Initiate physical therapy programs, instructing family members or caregivers involved in a patient's care, and participating in case conferences to discuss therapy goals. Provide training for patients in the use of prosthetic devices. Identify patient and family needs for additional home health services and refer as necessary. Prepare and submit clinical and progress reports outlining the achievement of goals. Contribute to discharge planning for patients. Provide consultation to home caregivers when appropriate. Conduct in-service education sessions for nursing personnel as needed. Participate in peer consultation processes. Supervise physical therapy assistants in accordance with organizational policy and state regulations. In cases where therapy is the only skilled service, instruct and evaluate home health aide care bi-weekly. The responsibilities mentioned here are meant to give an overview of the key tasks performed in this role. Additional job-related tasks may be assigned as necessary. POSITION QUALIFICATIONS Degree from an accredited baccalaureate or Master’s program in physical therapy. State licensure as a practicing Physical Therapist, as applicable. Minimum of two years of experience as a physical therapist; experience in community or home health settings is preferred. Excellent verbal, written communication skills, and strong organizational abilities. Current CPR certification is required. Must have a valid driver’s license and an insured, reliable vehicle that is in good working order.
