Hybrid RN Case Manager Hours Schedule is Monday-Friday 8:00am – 5:00pm EST. No nights, no weekends, no holidays and no on-call.1-2 days documentation at home, rest of the time in the field to patient home and/or facilities for assessmentsInterested Candidate must reside in or near Alexandria, Manassas, Fairfax, Arlington, Springfield, or surrounding areas) Hybrid RN Case Manager Compensation The pay for this position is $44.14 an hour paid weeklyBenefits are available to full-time employees after 90 days of employmentA 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates Hybrid RN Case Manager Responsibilities The Case Manager utilizes a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual's benefit plan and/or health needs through communication and available resources to promote optimal, cost-effective outcomes.Through the use of clinical tools and information/data review, conducts comprehensive assessments of referred member's needs/eligibility and determines approach to case resolution and/or meeting needs by evaluating member's benefit plan and available internal and external programs/servicesApplication and/or interpretation of applicable criteria and guidelines, standardized case management plans, policies, procedures, and regulatory standards while assessing benefits and/or member's needs to ensure appropriate administration of benefits.Utilizes case management and quality management processes in compliance with regulatory and accreditation guidelines and company policies and procedures Hybrid RN Case Manager Requirements RN License in VACase management or home health experience requiredExperience preferred managing chronic conditions, such as diabetes, hypertension, and asthma preferredCCM highly preferred If you think Hybrid RN Case Manager Position is a good fit for you, please reach out to me - feel free to call, e-mail, or apply to this posting
