Position Summary As a Certified Nursing Assistant Specialist, you will be instrumental in enhancing the quality of care for residents with chronic conditions. Working in collaboration with nurses, providers, and interdisciplinary teams, you will play a vital role in delivering proactive and comprehensive support, aiming to prevent avoidable hospitalizations and promote overall well-being. Key Responsibilities Resident Monitoring & Support: - Conduct routine vital signs checks, weight monitoring, and symptom assessments as per nursing directives. - Promptly observe and communicate any changes in residents' conditions to the licensed nursing staff. - Engage in chronic disease management through scheduled assessments for conditions such as CHF, COPD, diabetes, and hypertension. Documentation & Care Coordination: - Ensure accurate documentation of resident data in the electronic health record (EHR) and chronic care management workflows. - Assist nursing teams in gathering information for CCM reviews and care plan updates. - Facilitate timely communication regarding residents' needs and early warning signs. Resident Engagement: - Empower residents to take an active role in their chronic care plans, including hydration, mobility, medication adherence, and therapy participation. - Reinforce educational material as directed by nursing, such as diet reminders and activity prompts. Clinical Support: - Provide assistance to nurses and providers with chronic care procedures within the CNA scope. - Support telehealth check-ins and remote assessments by preparing residents and necessary equipment. Operational Support: - Collaborate with nursing leadership to enhance chronic care pathways and reduce hospital readmissions. - Help track follow-up tasks and scheduled chronic care engagements. Qualifications Valid Certified Nursing Assistant (CNA) license. Preferred experience in skilled nursing or long-term care settings. Strong knowledge of chronic disease symptoms and proactive care significance. Excellent communication, observation, and documentation capabilities. Adept at collaborating with interdisciplinary teams of nurses and providers. Compassionate and resident-centered approach to care. Physical Requirements Capability to lift and assist residents as necessary. Ability to stand, walk, bend, and engage in physical tasks throughout your shift.
