Job Summary As a Care Transition Specialist, you will play a vital role in facilitating seamless transitions of care for members from hospital settings to their homes or other facilities. Your focus will be on ensuring that members receive the best possible care during this critical time, ultimately aiming to reduce readmission rates. Essential Job Duties: Provide comprehensive support to members throughout a 30-day transitional care program, starting from hospital admission. Collaborate closely with hospital discharge planners, healthcare providers, and support networks to ensure safe transitions. Assess and facilitate transitions to ensure adequate caregiving, medical oversight, and functional support for members. Work with ancillary providers and public agencies to secure necessary services and equipment for smooth transitions. Conduct in-person visits during hospital stays and follow-up home visits for high-risk members post-discharge as needed. Utilize the Coleman Care Transition model to coordinate care and reassess member needs after discharge. Educate and empower members in seven key areas related to their transition of care, including medication management and recognizing worsening symptoms. Employ motivational interviewing techniques to support members in managing their health effectively. Identify barriers to care and provide personalized assistance to resolve issues. Lead interdisciplinary care team meetings to enhance collaboration. Offer guidance and education to non-behavioral health care managers as necessary. Be prepared to travel locally 40-50% of the time as required by state or contractual guidelines. Required Qualifications: A minimum of 2 years of healthcare experience, including at least 1 year in hospital discharge planning, care management, or behavioral health settings. Active and unrestricted licensure as a behavioral health clinician (LCSW, LPC, LMFT, etc.) in your state of practice. Valid driver's license, reliable transportation, and appropriate auto insurance for travel. Experience with or knowledge of the Care Transitions Intervention model. Strong understanding of community resources and support systems. Detail-oriented approach with proactive problem-solving skills. Ability to adapt and communicate effectively with diverse populations. Self-motivated with the ability to work independently and manage multiple priorities. Excellent verbal and written communication skills. Proficient in Microsoft Office Suite and related software. Preferred Qualifications: Certification in transitions of care or Certified Case Manager (CCM). Previous experience in hospital discharge planning or home health.
