Join Our Compassionate Team and Make a Difference! We are looking for a dedicated and empathetic Nurse Practitioner specializing in Palliative Care to enhance our multidisciplinary team. In this fulfilling role, you will deliver advanced palliative care to patients in their homes, ensuring they achieve the highest possible quality of life while navigating complex medical challenges. Work alongside the Palliative Medical Director and provide exceptional care to guide patients and their families through each stage of treatment. Key Responsibilities: Collaborate with the Palliative Medical Director and care team to provide comprehensive, patient-centered care. Conduct thorough patient assessments, including evaluations of physical, emotional, and psychosocial health. Develop and implement tailored care plans in partnership with patients and their families. Adjust treatment plans as necessary to meet patient needs and monitor progress. Prescribe medications and order diagnostic testing in accordance with state regulations. Ensure timely and accurate clinical documentation that complies with professional standards. Communicate effectively with physicians, caregivers, and other healthcare team members. Participate in evening and weekend on-call rotation as required. Facilitate referrals for additional care services when necessary. About You: Specialized Skills and Knowledge: Strong leadership and interpersonal skills, with excellent written and verbal communication abilities. Ability to make independent decisions and work autonomously. Knowledgeable about palliative medicine principles, home health services, hospice care eligibility, and Medicare/Medicaid regulations. Familiar with the interdisciplinary care model. Meets mandatory continuing education requirements for both Gentiva and state licensing boards. Proficient in using point-of-care software and technology. Education and Experience: Master of Science in Nursing from an accredited institution (or equivalent as per state law). A minimum of two years of experience as a Nurse Practitioner. Preferred specialization in Family or Adult Medicine. Experience with Part B billing is advantageous. Capability to work independently in an advanced practice setting. Licenses and Certifications: Current RN license and Nurse Practitioner certification as required by state law. A collaborative agreement with a supervising physician (or willingness to establish one). Part B Provider certification. Federal DEA license and state DEA license (if applicable). ACHPN certification in Hospice and Palliative Care is preferred. Valid driver's license and current automobile insurance.
