Pharmacist Job Specific Responsibilities Medication safety Patient safety monitoring, Overutilization monitoring system, drug management program reporting Collaboration & Communication Facilitates collaboration and communication between departments through various means, such as conducting and attending meetings, site visits, having one-to-one communications with operations management staff, and generally being visible and accessible to colleagues and staff. Special Assignments & Projects Coverage Determinations Participation in P&T committee Formulary development and submission to CMS Medication Therapy Management Pharmacy student preceptor FWA monitoring and reporting Coordinates delegates or performs special assignments and projects as requested by leadership staff. Pharmacist Qualifications and RequirementsExperience Required A minimum of three(3) years clinical pharmacy practice required. Good standing pharmacy License in Wisconsin. Preferred A minimum of one (1) year of pharmacy managed care and/or prescription plan administration preferred Education - PharmD or Bachelors of Science in Pharmacy Required. Must have active pharmacy license in the state of WI. Additional Requirements Knowledge of special pharmaceutical needs for the geriatric, chronic, mentally ill, or the physically and developmentally disabled adult. Certifications/LicensuresContinuing Education - Stays informed of current service provisions and legal issues. Performs other job-related duties as assigned.Additional Certification Information Good standing Pharmacist License in the State of Wisconsin Pharmacist work schedule: Monday through Friday 8AM-4:30PM, no holidays no weekends. Will be part of the RPH On-Call Rotation covering nights and weekends, every 5th week. Partly Remote position Supporting the dispensing pharmacy as assigned by the Pharmacy Manager including the on-call rotation.
