Social Worker A nursing home in Nashville is seeking a Social Worker to join their team. Social Worker Job Description: Maintaining and/or improving the resident's ability to control everyday physical needs and mental/psychosocial needs Identifying individual social and emotional needs Developing and maintaining individualized care plans Participating in the planning of the resident's admission, community re-entry or transfer to another facility Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident Work with the finance team to ensure the resident is educated and the proper documents are in place for the resident during their stay Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community Social Worker Qualifications/Requirements: BSW or MSW preferred, or Bachelors degree in the related field At least 3 years experience in the Social Work field required Ability to work flexible schedule PCC experience preferred An Equal Opportunity Employer
