Job Description: 75% Inpatient Palliative Care; 25% Outpatient Palliative Care Delivering comprehensive Inpatient and Outpatient Palliative Care services at Lima Memorial Hospital (LMH) Conducting patient consultations for symptom management, chronic disease management, advanced care planning, and hospice transition discussions Leading delicate conversations with patients and families regarding terminal diagnoses and care goals Performing inpatient rounds for palliative care and hospice patients, including ICU rounds Managing outpatient palliative visits through Oncology department, infusion center, Comprehensive Health Clinic, and Telehealth Collaborating with hospital specialists (ICU, Oncology, Pulmonology, Cardiology) to align care plans Providing independent care in a fast-paced hospital environment with backup from Bridge team in Findlay as needed Maintaining documentation in Meditech (inpatient) and eClinicalWorks (outpatient) Serving as a clinical expert in palliative care, educating staff and advocating for patient-centered care Managing up to 6 patient encounters per day over a 5-day workweek Not accepting Locums candidates Not accepting new graduate candidates - experience required Compensation and Benefits: Competitive base salary based on experience and training (to be discussed at interview) with quality bonus potential.
