About the Job All Care and Comcare Home Health is currently seeking a qualified Physical Therapist to join our dedicated healthcare team. This position offers a unique blend of administrative oversight and hands-on patient care, presenting an excellent opportunity for a PT looking to expand their professional scope. Position: Field Physical Therapist Key Responsibilities:Provides comprehensive, skilled physical therapy services/interventions in accordance with physician orders. Comfortable with start of care (SOC), Evaluations, Resumption of Care (ROC), Recertification (Recert), and Discharges (DC). Assesses patient skilled needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions. Collaborates with the interdisciplinary team to plan, evaluate team's goals for each individual patient. Educates, instructs, and trains patient, family, responsible caregiver on patient's physical therapy program. Qualifications:Active/Registered by the Physical Therapy Board of California. Graduate from a Physical Therapy accredited school. Strong documentation skills. Excellent communication and interpersonal skills. Willingness to travel to patient locations for evaluations.
