With over 400 employees in Southern Colorado, you would be joining a mighty team of support and administrative staff, case managers, clinicians, physicians, nurses, and others in our efforts to improve the health and wellbeing of our community. We're looking for a Clinical Care Specialistto join our Adult Outpatient team. What You'll Be Doing In this role, you will provide behavioral health counseling and group treatment while offering resource-based case management to support clients' mental health and substance use needs. You'll collaborate with treatment providers to deliver coordinated, team-based care and help clients identify natural supports and effective coping mechanisms. You will teach and strengthen skills related to symptom management, problem-solving, independent living, and socialization. Additionally, you'll develop, implement, and review individualized treatment plans, maintain proficiency in the electronic health record system, and perform other duties as assigned by leadership. Specifically, the Clinical Care Specialist: Makes appropriate referrals to address mental health and substance use needs Provides behavioral health counseling, group treatment, and resource-based case management Collaborates with other treatment providers to ensure a team-based approach to treatment, when applicable Identifies coping mechanisms and natural supports that may be available Teaches and enhances client coping, symptom management, problem-solving, independent living, and socialization skills Develops, implements, and reviews treatment plans with clients Maintains competency and proficiency with our electronic health record system Performs other job-related duties assigned by the director or his/her designee Desired attributes: Is adaptable to change in the workplace and uses change as an opportunity for innovation and creativity Takes ownership of problems, has ability to brainstorm different problem resolution paths, uses sound judgment in selecting solutions to problems, and demonstrates consistent follow through Has job knowledge and skills to perform the fundamental job functions and is able and willing to assume greater responsibility over time regarding the scope of work Has the ability to inspire and model collaborative teamwork Demonstrates an understanding of customer service regarding accommodation, politeness, helpfulness, trust building, appropriate boundaries, and flexibility Physical requirements include the following: Frequently stays stationary for prolonged periods of time. Frequently moves within and between facilities. Frequently exposed to stressful or emergent situations. Frequently operates a computer and other office equipment to record health data. Constantly communicates with clients and others. Occasionally moves equipment and/or materials up to 20 pounds.
