Chicago Healthcare Job Details Location: Chicago, IL (Northside, Far North & Suburbs, Southside & Suburbs - specify your preferred region in your application) Employment Type: Travel/Direct Hire Healthcare Setting: 80% field/community (patient homes), 20% remote (home/office) Schedule: Monday - Friday, 8:00am - 5:00pm Shifts per Week: 5 Shift Duration: 8-hour days Contract Length: 13 weeks, with potential for extension Estimated Weekly Pay: $2260 - $2430 LCSW/LSW Job Requirements Active LCSW or LSW license in Illinois (RN with strong case management experience also considered) Minimum 1 year of experience working with Medicaid, low-income, or disadvantaged adult populations Proven case management experience in healthcare or clinical settings Comfortable conducting community visits and entering patient homes (80% field work) Strong leadership and clinical support skills to drive positive outcomes for members Reliable vehicle, valid driver's license, and current auto insurance required No new graduates; experience with behavioral health populations preferred Healthcare Responsibilities Deliver comprehensive care management and clinical support to adult Medicaid and Medicare Advantage members Conduct in-home and community visits to assess patient needs, develop care plans, and coordinate healthcare services Collaborate with interdisciplinary healthcare teams to optimize member health outcomes Participate in daily team meetings at 8:00am and 4:30pm; punctuality is essential Document all member interactions and interventions accurately and timely Lead team efforts to support member success (no direct reports) Advocate for members and connect them to vital community resources Apply now to join our Chicago healthcare team and advance your LCSW career in Illinois.
