In this role, as a Hospice Visiting Nurse (RN), you will: Support patients, families, and loved ones during their end-of-life journey. Conduct comprehensive assessments to establish goals for symptom management and end-of-life care using effective case management strategies. Collaborate within an interdisciplinary team that approaches patient needs holistically. Build and maintain therapeutic relationships through effective communication with patients and families. Utilize state-of-the-art technology to document the skilled services provided. Qualifications: Registered Nurse with a valid CT RN license. Minimum one year of RN experience; Homecare or Hospice experience preferred. Valid driver's license and reliable transportation required. Computer proficiency is essential.
