Your recruiter will provide the most up-to-date details during the hiring process.How you'll make an impact in this roleAscension Illinois is looking for a Board Certified/Board Eligible Advanced Practice Provider, Nurse Practitioner or Physician Assistant for our Family Medicine Department.This is a great opportunity for the right candidate to partner with Ascension Illinois to provide medical services to patients.Position Highlights: 100% outpatient Full-spectrum Family Medicine Patient friendly hours, 1 Saturday per month and 1 evening per week See kids-adults Very busy Family Medicine Practice Nice mix of patients and cases MA support Excellent office support staff for other clinical tasks Great Northwest Suburban location Responsibilities: Provide direct diagnostic, therapeutic, and preventive health care services under the supervision of a physician. Obtain patient history and perform assessment. Order, perform and interpret diagnostic procedures. Provide and document direct treatment and management of health conditions. Provide counseling and education of patients and their families/caregivers concerning preventative health, treatment options and community resources. Perform preventative health assessments, screening, immunizations and care.
