Job Description Hospice Social Worker A respected community hospital in Arkansas, just outside of Branson, Missouri, is seeking a Hospice Social Worker to join their Home Health & Hospice team. Offering $24.45 - $33.00/hour + $5, 000 Sign-On Bonus Hospice Social Worker Opportunity: Full-Time, Permanent, Direct-Hire, Opportunity Monday - Friday, with rotating weekend on-call Offers a comprehensive and robust benefits package which includes the standard medical, dental, and vision coverage, as well as, Tuition Reimbursement, Disability coverage, and much more Hospice Social Worker Responsibilities: Be a core member of the Hospice team and provide support to patient's and family's emotional, social, and economic needs relating to the impact of a terminal illness Plan the transition and transportation of patients from hospitals to a home or another facility, arrange services, collaborate with doctors, nurses, and insurers Plan and Lead interdisciplinary meetings, facilitate the referrals for community resources, and review critical access admissions and monitor patients with regularly scheduled admission visits Hospice Social Worker Qualifications: Must have an Arkansas LCSW or LMSW or be eligible to obtain one Must have or be eligible to obtain BLS Certification Surrounding Location & Community: Centrally located community within proximity to areas for activities like fishing, hiking, camping, and more.
