Job Description

Cardiac Rehab Nurse This is a 24-hour position, typically scheduled as three 8 hour shifts per week. The Cardiac Rehab Nurse, under the general supervision of the Nursing Director, functions as an ambulatory practice nurse for the Cardiac Rehab Program. Responsibilities include: Provide nursing care for patients during cardiac rehabilitation, PAD supervised exercise and cardiac lifestyle programs. Organize, prioritize and manage your own panel of 20-30 patients, with support from a multidisciplinary care team of exercise physiologists, nutritionists, and cardiologists. Perform initial intake interviews for patients interested in the program. Develop a personalized treatment plan for each patient. Oversee patient treatment plans and provide expertise in case management including cholesterol and dietary management, smoking cessation, and behavior/lifestyle modifications. Supervise exercise sessions. Use telemetry to monitor and record patients' heart activity for inappropriate responses. Collaborate with team Exercise Physiologist to prescribe ACSM protocols for exercise in the cardiac population. Maintain a knowledge of ACSM exercise prescription protocols. Essential functions include maintaining accurate, detailed reports and records, administering medications to patients and monitoring patients for reactions or side effects, recording patients' medical information and vital signs, monitoring, recording, and reporting symptoms or changes in patients' conditions, consulting and coordinating with healthcare team members to assess, plan, implement, or evaluate patient care plans, and modifying patient treatment plans as indicated by patients' responses and conditions. Qualifications include an associate's degree in nursing or a bachelor's degree in nursing preferred, clinical nursing experience 0-1 year required, familiarity with the principles and skills needed for practical nursing to provide patient care and treatment, knowledge of the care required by respective age groups for which care is being provided, ability to maintain confidentiality and secure sensitive information, knowledge of medical terminology, excellent verbal and communication skills, and ability to accurately screen and triage acute patients. Additional job details include onsite work location at 25 New Chardon Street, scheduled weekly hours of 24, regular employee type, day work shift in the United States of America, and a pay range of $41.72 - $105.65/hourly.
