Under the direction and supervision of a Registered Nurse, the LPN plays a vital role in supporting patient health, independence, and quality of life. In this role, you will collaborate closely with physicians, RNs, therapists, social workers, patients, and their families to carry out individualized care and treatment plans. By combining clinical skill with strong communication and teamwork, the Home Health LPN helps ensure that each patient receives safe, effective, and patient-centered care tailored to their unique needs. The LPN reports directly to the Nurse Manager and functions as an integral member of the home health care team. ResponsibilitiesDirect Patient Care Provide hands-on nursing care in accordance with the State Nurse Practice Act and under the direction of a Registered Nurse. Perform vital signs monitoring and complete head-to-toe patient assessments during each visit. Deliver high-quality wound care, including surgical wound management and operation of wound vac systems. Assess, manage, and perform care for central and peripheral lines, urinary catheters, colostomies, PleurX drains, and laboratory draws. Care Plan Implementation Implement the individualized plan of care initiated by the Registered Nurse. Provide timely updates regarding patient status and changes in condition. Participate in interdisciplinary case conferences to support coordinated patient care. Patient & Family Education Assess the education needs of patients and caregivers. Provide clear instructions on self-care techniques, disease management, and treatment plans. Ensure education is understood and documented thoroughly. Clinical Documentation Complete accurate and timely documentation aligned with the plan of care and regulatory requirements. Maintain clear and organized records of assessments, interventions, responses, and communications. Equipment & Supply Management Prepare and use equipment and supplies safely, efficiently, and according to aseptic technique. Adhere to infection prevention standards while performing all clinical procedures. Professional Collaboration & Duties Collaborate with physicians, nurses, therapists, social workers, and other members of the care team to support patient goals. Perform additional duties as assigned to support patient care and operational needs. QualificationsPrior experience as an LPN in a skilled nursing facility or in the home care setting preferred Credentials LPN - Licensed Practical Nurse BLS - Basic Life Saving certification (American Heart Association) EducationGraduate of an accredited LPN program Entry USD $24.62/Hr. Max USD $38.16/Hr.
