Key Responsibilities: Patient Care: Review and monitor patient medication regimens to ensure appropriate therapeutic outcomes. Work with the healthcare team to develop and adjust medication therapies. Consultation: Provide medication counseling to patients and healthcare providers, ensuring safe and effective use of medications.
