Primary Care Nurse Practitioner- Full Time Location: Salisbury, MD and surrounding areas Salary Range: $100,000 to $145,000 Annually Schedule: 4-day, 36-hour workweek (Monday-Thursday!) Position Summary The Primary Care Nurse Practitioner (NP) delivers comprehensive, person-centered primary care services to individuals across community, home, and office settings. This full-time, community-based role focuses on improving health outcomes through assessment, diagnosis, treatment, health promotion, and care coordination. The NP serves as a key member of an interdisciplinary team, contributing to continuity of care and supporting clients whose physical health needs may be influenced by mental health conditions, lifestyle factors, or social determinants of health. Essential ResponsibilitiesClinical Care & Service Delivery Provide scheduled primary care services including physical assessments, diagnosis, and medication management to support clients' health and wellness across home, community, and office settings. Utilize the electronic medical record to document visits, coordinate care, and ensure completion of all required billing elements. Deliver community-centered, respectful, and person-centered care that promotes positive health behaviors and wellness. Collaboration & Team-Based Care Participate in interdisciplinary team meetings to support treatment planning, care coordination, and integrated service delivery. Collaborate with behavioral health providers, rehabilitation staff, and community partners to address acute and chronic health needs, especially when impacted by mental health conditions, lifestyle, or environmental factors. Compliance, Ethics & Professional Standards Maintain accurate, timely, and high-quality clinical documentation that meets agency, state, and accreditation standards. Uphold all ethical and legal requirements, including HIPAA, CARF standards, and Maryland state regulations, ensuring strict confidentiality and professional boundaries. Participate in required training, continuing education, and professional development activities. Operational Responsibilities Provide services at assigned locations and, when applicable, collaborate with partner agencies under contract. Perform additional responsibilities as assigned by the ACT Vice President or the Vice President of Psychiatric Rehabilitation. Qualifications Master's-level training and certification as a Family Nurse Practitioner; must hold current Maryland licensure and be fully insurable. Current CPR and First Aid certification preferred. Valid driver's license with a safe driving record for travel to home and community sites. Strong verbal and written communication skills and the ability to work effectively in a multidisciplinary environment. Why Join HealthPort?
