Location: San Jose, CA Facility: Hospital Start Date: Asap Length: 13 weeks Available Shifts: 08:00 AM to 04:30 PM Role Type: Care Critical PAY RATES: Local: $70/hr. Travel with housing: $53/hr Travel: $28/hr. taxable + $1960/wk. non taxable - 28/49 Must be 50 miles away from permanent residence to qualify for housing allowancePreferred Qualifications: 1+ year experience as Patient Care Coordinator (PCC) / Case Manager Previous Kaiser Permanente experience preferred EPIC charting experience Experience with Medicaid/Medicare and authorizations preferred RN Responsibilities and Duties : Coordinate and oversee care progression for acute inpatient / critical care patients, ensuring safe, timely, and cost-effective outcomes. Collaborate daily with physicians/rounding teams, bedside nursing, utilization review, social services, and interdisciplinary team members to support the patient's plan of care and clinical milestones. Participate in rounds/huddles, communicate updates, identify barriers to discharge, and drive next steps to maintain continuity of care. Develop, evaluate, and coordinate a comprehensive discharge and transition plan with the patient/family and care team (including post-acute needs and follow-up care). Coordinate transfers (e.g., step-down, SNF/LTAC, rehab, outside facilities), ensuring appropriate level-of-care recommendations and smooth handoffs. Obtain and track authorizations/approvals and coordinate with internal departments and external providers/facilities as needed for post-acute services and discharge needs. Provide patient and family support through education, goal-setting, and navigation of care plans, ensuring understanding of the plan and next steps. Maintain accurate documentation of assessments, plans, care coordination activities, and discharge arrangements in accordance with Kaiser/KFH policies and regulatory requirements. Monitor progress and communicate changes in patient status, risk factors, and discharge readiness to appropriate stakeholders to prevent delays and avoidable utilization. Serve as a key liaison to ensure the continuum of care from inpatient to outpatient/post-acute settings, reducing readmissions and supporting quality outcomes.
