Wellness Nurse (LVN/LPN)Position Summary The Wellness Nurse (LVN/LPN) supports a proactive, resident-centered approach to health and wellness by coordinating clinical care, monitoring changes in health status, and partnering closely with residents, families, and the clinical leadership team. This role helps promote safety, independence, and quality of life while ensuring compliance with applicable healthcare standards and regulations. Essential Responsibilities Resident Wellness & Care Coordination • Monitor ongoing resident health needs to support safe, high-quality care and services. • Assist with resident assessments and data collection at move-in, 30 days post move-in, semi-annually, and with significant changes in condition. • Conduct monthly wellness visits and document observations according to community standards. • Follow up after falls, incidents, or changes in condition to ensure timely communication, documentation, and care coordination. • Maintain accurate wellness records including vital signs, weights, and other required clinical information. • Communicate effectively with residents, families, and responsible parties regarding health-related updates and care needs. • Coordinate communication with attending physicians as appropriate. • Maintain emergency supplies and clinical inventory for the community. • Provide clinical support and guidance to team members as needed. Infection Prevention & Resident Safety • Follow infection prevention and control policies and procedures. • Partner with leadership and team members to support infection control practices, training, and competency development. • Practice universal precautions and proper handling/disposal of bloodborne pathogens, bodily fluids, sharps, and hazardous materials. Medication Oversight • Monitor medication and treatment profiles to ensure services are administered and documented accurately. • Transcribe physician orders into Medication Administration Records (MAR) accurately and promptly. • Conduct regular MAR audits to ensure accuracy and compliance. • Demonstrate knowledge of the Six Rights of Medication Administration. • Serve as liaison with pharmacy partners to support efficient medication services. • Maintain medication rooms, carts, and treatment areas in an organized, clean, and compliant manner. • Conduct medication observations and skills checks for medication technicians. Quality Assurance & Regulatory Compliance • Support a resident-centered care model focused on quality outcomes and continuous improvement. • Participate in clinical and quality assurance meetings. • Maintain knowledge of applicable federal, state, and local regulations related to resident care and services. • Assist with implementation and monitoring of clinical documentation standards. • Communicate resident changes, concerns, and recommendations to clinical leadership in a timely manner. • Assist in ordering medications and equipment as needed. • Maintain required certifications and complete all required training. • Support resident and team member engagement initiatives. • Perform other duties as assigned. Qualifications • Active and unrestricted LVN/LPN license in the State of California. • Experience in senior living, assisted living, memory care, or related healthcare settings preferred. • Understanding of medication administration and clinical documentation standards. • Ability to work collaboratively within an interdisciplinary team.
