Job Description

Shift is typically 8am-5pm, Monday-Thursday; Friday 8am-3pm. No call is required, no evenings or weekends required. There is possibility off working alone, if the staff Physical Therapist has a day off.Minnesota Physical Therapist license is needed. Require Physical Therapist licensure with graduation from program. No minimum years of experience. 1.General job duties. Types of procedures required. Provides day-to-day work guidance for a team or work group often in a shift and/or in the completion of tactical processes. Uses a combination of knowledge and experience to provide technical direction. Performs staff work in their area of expertise along with providing formal leadership and provide patient/resident care to meet department needs. Plans, organizes and delivers specialized therapy programs that help clients/patients/residents recover and improve their mobility, relieve pain, and increase strength. Conducts examinations, evaluations, and interventions patients/residents who are affected by injury or disease. Utilizes tools and techniques to administer therapy interventions following safeguards. Documents client's/patient's/resident's information for evaluation; establishes therapeutic plans and modifies them if necessary. Educates clients/patients/residents and their family members about treatment plans/schedules and self-exercises to be continued at home. Knowledge of and utilizes appropriate age-related treatments and evaluations relating to the needs of clients/patients/residents. Demonstrates and incorporate knowledge of current research into daily treatment of clients/patients/residents. May treat individuals of all ages with varying diagnosis and disorders. Will demonstrate documentation in accordance with departmental guidelines and meet all regulatory requirements. Demonstrates level of treatment associated with their specialty provided to their clients/patients/residents. Follows and adheres to accepted clinical practice guidelines of professional organization. 2.Equipment utilized in the unit/dept. (Is there computerized charting, medication systems, etc.) EPIC Charting 3.Required skills and minimum years of experience. 4.Specific licensures and certifications/registrations required. (ARRT, CRT, BLS, ACLS, etc) Licensed Physical Therapist 5.Shift or schedule requirements. (Will the employee ever work alone? Is call required? If so, how often? Are weekends required?) Monday through Friday 8am-5pm. Sole physical therapist in facility. No call/weekends required. 6.Floating to other units/departments. (If so, to which units/departments?) Provides inpatient and outpatient physical therapy 7.Bed size/number of rooms for this unit/dept. 4 outpatient exam rooms, 14 bed inpatient Med Surg unit 8.Tell us about the unit/department specific training and orientation program. Physical therapy assistant will help train/assist with orientation - All clinicians must badge in and out at facility - Scrub Color: Polo (Ceil Blue, Khaki, Gray, Chocolate Brown) or business casual.MUST note in submission if clinician has ever worked for Sanford Health. If clinician previously worked with Sanford, they must have an end date 6 months prior to their submission date.
