Job Summary The Clinical Leader Participates in planning, organizing, implementing, and evaluating patient care and developing programs to support clinical care consistent with the mission and goals of Brigham & Women's Faulkner Hospital as well as the requirements of JCAHO and other applicable federal, state, and local regulatory and/or accrediting agencies. Responsibilities include participation in oversight and operation of the patient care area, ensuring quality patient care for patients in the following age groups: pediatrics, adolescents, adults, and geriatrics. The Clinical Leader will also ensure the practice of the Patient and Family Centered Care Model in an environment that exemplifies best practice. 1. PROVISION OF A SAFE ENVIRONMENT • Anticipates and identifies safety risks and takes appropriate action. • Assures compliance with safety standards by assessing staff and patient needs and taking appropriate action. 2. ORGANIZATIONAL ETHICS • Maintains and promotes an atmosphere which reflects the mission and values of the organization. Respects and supports a culturally diverse staff and patient population. • Acts as a patient advocate: supports and implements procedures for resolving ethical conflicts and patient rights. 3. COORDINATOR OF RESOURCES • Staffs appropriately based on census and acuity. Utilizes resources to meet Departments educational needs. 4. COORDINATOR OF INFORMATION • Assures comprehensive documentation based on unit-specific and hospital-based criteria. Evaluates documentation and follows up with staff • Demonstrates a working knowledge of hospital information systems by ensuring cross-departmental communication of organizational information. • Maintains a system for communication with manager and staff of assigned units. • Meets deadlines for projects and reports. COLLABORATIVE INTERDISCIPLINARY CARE • Fosters collaboration of health care team members to provide interdisciplinary patient care. • Participates in unit, department and/or interdisciplinary committees. • Role models effective communication, teamwork and mediations as appropriate. PATIENT/FAMILY EDUCATION • Supports and facilitates patient/family age appropriate education into orientation, in-services, and continuing education. Develops and implements patient/family teaching protocols and materials if appropriate. COMPETENCY • Identifies trends and competency needs of staff based on staff input. • Supports compliance with hospital, nursing, and unit specific competencies (orientation and annual) by developing a system for monitoring competencies. Coordinates with Nursing Director and staff development to provide method and content for annual competencies. • Evaluates competency of non-licensed personnel in collaboration with Nursing Director. QUALITY IMPROVEMENT • Demonstrates knowledge of principles of SEAQ /performance improvement and customer service. Participates in development and implementation of the QI plan for the area. • Supports and revises patient care based on QI results and current research data. • Ensures quality assurance in all aspects of patient nursing care. PATIENT CARE • Acts as a clinical resource to staff by: Evaluating patient care outcomes based on age-specific needs; Recommending changes in practice based on current research data; Developing and promoting patient care standards • Assists staff to problem solve and respond to psychosocial needs of patients and families and takes appropriate action. CONTINUUM OF CARE • Facilitates patient information across departments, systems, and other facilities.
