Job Type Full-time Description The Care Manager serves as the "Systems Expert" within a primary care-based Center of Excellence for Opioid Use Disorder. This role bridges the gap between clinical care and patients' lives outside the office, providing comprehensive care coordination, insurance navigation, and social support services. The Care Manager works collaboratively within a multidisciplinary team to support recovery, stability, and improved health outcomes. We operate on a 35 hour work week (Monday to Friday 9AM - 4PM) and offer an excellent benefit package: 100% employer paid medical insurance 20 days of PTO starting with your first full calendar year, PTO prorated at time of hire 9 company holidays 401k Plan with 100% match on the first 3%, 50% match on additional 2%, vested immediately 100% Paid Medical and Parental Leave Paid CEs - if applicable Monthly SEPTA pass with minimal EE contribution The anticipated annual salary range for this position is $50,000-$75,000. Compensation is determined based on experience and qualifications. In certain circumstances, offers may fall outside of the listed range. Assessment & Care Planning Conduct ASAM Level of Care and Social Determinants of Health (SDOH) assessments. Develop, implement, and monitor individualized Care Management Plans aligned with patients' recovery goals. Reassess needs regularly and adjust plans accordingly. Maintain accurate and compliant documentation in the electronic health record. Benefits & Insurance Navigation Assist with Medicaid applications, renewals, and redeterminations. Educate patients on insurance benefits and eligibility. Resolve coverage barriers to ensure uninterrupted access to MOUD and other services. SDOH Support & Resource Connection Support access to housing, transportation, food assistance, employment, education, and specialty medical care. Connect patients to community resources and ensure follow-through. Care Coordination & Advocacy Collaborate with medical providers, recovery specialists, mental health clinicians, and community partners. Serve as liaison with external systems (e.g., probation/parole, housing authorities, CYS). Advocate to reduce systemic barriers and promote equitable access to care. Requirements MSW required; BSW or related degree with significant relevant experience considered. Experience with substance use disorders, co-occurring conditions, and complex social needs preferred. Knowledge of ASAM criteria, harm reduction, recovery-oriented systems, and trauma-informed care preferred. Strong communication, documentation, and advocacy skills. Ability to build rapport with diverse populations and thrive in a multidisciplinary team environment. Strong communication skills, including the ability to advocate effectively on behalf of patients while maintaining professional relationships with external agencies. Proficiency in MS Office and electronic health records (eCW preferred). Cultural humility and ability to work effectively with individuals from diverse socioeconomic, cultural, and racial backgrounds. Active Listening - Giving full attention to what other people are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times. Speaking - Talking to others to convey information effectively. Writing - Communicating effectively in writing as appropriate for the needs of the audience. Ability to use computers, keyboards and mouse to enter, access or retrieve data. Ability to use photocopiers, scanners.
