Position Overview: The primary purpose of the Licensed Vocational Nurse Case Manager is to develop, evaluate, implement health care plans for individual patients, manage cost effective medical plans, while providing the highest level of patient care. The Case Manager also communicates treatment plans between families, doctors, providers, and patients. Sign-on bonus terms and amounts are subject to change and are based on the employee's date of hire. Essential Duties: Evaluate and update patient health care plans. Must do rounds with the Doctors/Medical Directors. Assessment for new patients. Review test results from medical exams. Implement Physician orders and follow up with treatment plans. Documentation of medical records. Provide emotional support to families and patients. Provide education about health care plans to patients and families. Act as the main liaison between the SNF IDT team and Kaiser case managers, physicians, and utilization management staff. Communicate with families, health providers, and patients. Coordinate admissions, continued stays, and discharges for Kaiser members. Participate in care conferences and IDT team meetings. Monitor patient progress and communicate changes in condition to Kaiser MDs. Support quality improvement initiatives and reduce avoidable re-admissions. Qualifications: Must possess, as a minimum, a RN Nursing Degree from an accredited college or university. Must have, as a minimum, one (1) year of experience with Case Management in a SNF or Hospital. Must have training in rehabilitative and restorative nursing practices. Effective communication, excellent customer service, teamwork, and organizational skills. Must possess a current, unencumbered, active license to practice as an RN in this state. Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served. Being a supportive team member, contribute to and be an example of teamwork and team concept. Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public. Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities. This position requires the successful completion of a background check, pre-employment physical, and drug screening as part of the hiring process.
