Home Health Occupational Therapist (OT)We are seeking a compassionate and motivated Home Health Occupational Therapist (OT) to join our growing team. This role provides skilled occupational therapy services to patients in their homes, focusing on improving independence, safety, and quality of life.The Occupational Therapist evaluates patient needs, develops individualized plans of care, and delivers evidence-based treatment in accordance with physician orders, agency policies, and regulatory standards.Key Responsibilities Evaluate patients to determine functional limitations, treatment needs, and prognosisDevelop and implement individualized occupational therapy plans of careProvide skilled occupational therapy services in the home settingAddress Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs)Educate patients and caregivers on treatment plans, safety, and adaptive techniquesRecommend and assess adaptive equipment and home safety modificationsReassess patient progress and modify treatment plans as neededCoordinate care with the interdisciplinary healthcare teamParticipate in discharge planning and discharge patients when goals are metSupervise Home Health Aides per agency policy and regulatory requirementsMaintain accurate, timely, and compliant clinical documentationAdhere to professional, ethical, and regulatory standards of practice Qualifications Graduate of an accredited Occupational Therapy programCurrent Occupational Therapist (OT) license in the state of practiceMinimum of one (1) year of Occupational Therapy experience; home health preferredKnowledge of home health regulations and documentation requirementsStrong clinical assessment and organizational skillsExcellent verbal and written communication skillsAbility to work independently and manage a flexible scheduleAbility to legally work in the United States Physical Requirements Ability to travel between patient homesAbility to lift, transfer, and assist patients as neededAbility to stand, walk, bend, and reach during patient careAbility to communicate effectively with patients and healthcare team members Equal Opportunity Employer We are an Equal Opportunity Employer and are committed to providing a workplace free from discrimination.
