1st TIMER/ 1 YR OK 5x8's 0800-1630 M-F NO Call required Occasional weekends required Home Care treatment. Our patient population varies- patients with Neuro conditions, Musculoskeletal conditions, Cardioplumonary conditions, Orthopedic conditions Requirements: Home Health, Home exercise, lifting and transfer, patient family educations, safety and injury prevention, scheduling, signs/symptoms of physical/sexual/psychological abuse. Documentation Experience: assessments, discharge, evaluations, long term goals, OASIS, plan of care, Reassessment, Short term goals, time management Equipment: Gait Belt, Roller walker, standing walker, supplemental O2, Certifications: BLS, IN License (in-hand to sub) Must have valid Drivers License and updated auto insurance. 1. Travel Radius - We cover 10 counties and the PT will cover much of the Northwest area. We have a scheduler that will help with assignments so they will not be driving too far. 2. In-Office Requirement: The orientation will be in-office (possibly about 1 week or 1.5 weeks the most). Most of our meetings like case conferences and huddles are virtual. 3. Will reimburse mileage. 4. Average visits per day/point system - 25-28 points per week. Each visit has a weight: like routine visit/ discipline d/c is 1.0; evals/ functional RA is 1.5; admission is 2.5 points.
