Payrate: $56.14/hr + mileageRequired Availability: Full-Time | Monday-Friday, 8:00 AM - 5:00 PM Caseload Benchmark: 6 patients per day. RN FIELD CASE MANAGER | SUMMARY AND HIGHLIGHTS:The RN Field Case Manager will p erform comprehensive assessments of high-risk members and collaborate with primary care providers to implement individualized, comprehensive care plans. The goal will be to coordinate care transitions, identify opportunities for health promotion, and act as a strong advocate for patient privacy and clinical excellence. This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needsBenefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insuranceA 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates RN FIELD CASE MANAGER | RESPONSIBILITIES: Assessment & Care Planning: Perform comprehensive assessments of high-risk members and collaborate with PCPs to implement individualized care plans.Coordination of Care: Manage care transitions by coordinating with physicians, social workers, discharge planners, and claims professionals.Resource Management: Identify and implement non-medical supports (e.g., housing, transportation) to ensure compliance with treatment plans.Outcome Tracking: Maintain detailed records of clinical, functional, and fiscal outcomes.Advocacy: Act as a patient advocate while ensuring strict adherence to privacy and confidentiality standards. RN FIELD CASE MANAGER | REQUIREMENTS: Education: Associate's or Bachelor's Degree in Nursing (ASN or BSN).Attendance: Commitment to mandatory attendance during the first 90 days.Licensure: Current, unrestricted State Registered Nurse license (Compact license accepted).Certification: Case Management certification is required.Skills: Strong ability to analyze complex medical information and make rational, cost-efficient clinical decisions.Preferred Experience: Prior experience in Medical-Surgical (Med/Surg) or Critical Care environments.
