Join Our Mission: Provide Compassionate Care and Empower Lives. We are seeking a dedicated and empathetic Nurse Practitioner specializing in Palliative Care to join our interdisciplinary team. This fulfilling position allows you to deliver advanced palliative care directly to patients in their homes, enhancing their quality of life while expertly managing complex medical scenarios. You will work independently, forming a close partnership with the Palliative Medical Director to provide exceptional care and support to patients and their families during every phase of treatment. Key Responsibilities: Collaborate with the Palliative Medical Director and care team to offer comprehensive, patient-centered palliative care. Perform detailed patient assessments that encompass physical, emotional, and psychosocial dimensions. Engage with patients and families to develop and execute tailored care plans. Monitor and adjust treatment plans as patient needs evolve. Prescribe medications, including controlled substances, and order necessary diagnostic tests in line with state regulations. Maintain prompt and accurate clinical documentation that adheres to professional standards. Communicate effectively with physicians, caregivers, and other healthcare professionals. Participate in evening and weekend on-call rotations as needed. Coordinate additional referrals for care services when necessary. About You: Specialized Knowledge/Skills: Exemplary leadership and interpersonal skills, along with strong written and verbal communication abilities. Self-motivated decision-maker capable of working independently. Understanding of palliative medicine principles, home health, hospice eligibility, and Medicare/Medicaid regulations. Familiarity with the interdisciplinary care model. Commitment to ongoing education requirements for Gentiva and licensing bodies. Proficient in point-of-care technology and software usage. Education/Experience: Master of Science in Nursing from an accredited institution (or equivalent as per state law). A minimum of two years' experience as a Nurse Practitioner. Preferred specialization in Family or Adult Medicine. Experience with Part B billing. Ability to operate autonomously in an advanced practice setting. Licenses/Certifications: Current RN license and Nurse Practitioner certification as required by state regulations. Collaborative agreement with a supervising physician (or a readiness to establish one). Part B Provider certification. Federal DEA license and state DEA license (if applicable). ACHPN certification in Hospice and Palliative Care is preferred. A valid driver's license and current automobile insurance.
