Physical Therapist- Homecare

Job Title: Physical Therapist - Homecare Location: Orlando, FL Payrate: $48 - $61/hr Summary: Seeking a licensed Physical Therapist to provide skilled home health services. Responsibilities include comprehensive evaluations, developing and managing Plans of Care (POC), and delivering evidence-based interventions to improve mobility, reduce pain, and promote independence. The role requires compliance with Medicare guidelines, timely documentation, and interdisciplinary collaboration. Key Responsibilities:Evaluation & Planning Conduct comprehensive initial evaluations (strength, balance, gait, home safety, etc.) Develop individualized, measurable Plans of Care in collaboration with physicians and care teams Ensure timely physician certifications and recertifications Treatment & Intervention Deliver skilled physical therapy (therapeutic exercise, gait training, manual therapy, etc.) Educate patients/caregivers on transfers, home exercises, fall prevention, and equipment use Adjust plans based on progress and clinical judgment Participate in discharge planning when appropriate Documentation & Compliance Complete documentation within 24-48 hours per Medicare and company standards Ensure all records support medical necessity and skilled need Use standardized outcome tools (e.g., TUG, BERG, LEFS) Participate in quality audits and training Collaboration & Communication Coordinate care with interdisciplinary team members Attend case conferences and communicate patient updates promptly Requirements: Degree in Physical Therapy (DPT, MPT, or BSPT) Active PT license in the state of Florida. CPR/BLS certified (Required) Minimum 1 year of clinical experience preferred; home health experience strongly preferred.
