Position Title: Physical Therapist Location: Lakeport, CA 95453 Long Term Contract Reports To: CCS Administrator / Designee The Physical Therapist (individual or vendor-proposed resource) will provide therapeutic services for CCS-eligible children referred to the Medical Therapy Unit (MTU). The role involves comprehensive evaluation, development of treatment plans, delivery of professional PT services, documentation, caregiver education, and coordination with County staff, schools, and medical professionals. The PT will comply with all Federal, State, County, and CCS regulations while ensuring patient-centered, effective, and timely therapy. Key Responsibilities Clinical Responsibilities Evaluate and treat CCS clients referred to the MTU. Perform standardized and non-standardized assessments (ROM, muscle strength, mobility, ADLs, posture, reflexes, sensory, motor, oral motor, perception, respiratory). Write formal evaluations (every 6 months for frequent clients; every 12 months for less frequent clients, or when service levels change). Recommend therapy frequency, duration, and intensity. Deliver PT services according to physician-directed Plan of Treatment. Maintain accurate documentation (daily treatment notes, monthly progress notes, Patient Therapy Records). Educate/instruct caregivers in home exercise programs and provide written guidance. Assess and recommend postural supports, splints/braces, orthotics/prosthetics, and durable medical equipment (DME). Conduct home visits when required to evaluate DME needs. Consult with DME providers and participate in orthotic/prosthetic evaluations. Attend IEP meetings to present therapy goals and service recommendations. Participate in Medical Therapy Conferences to present evaluations and therapy recommendations. Administrative / Compliance Responsibilities Adhere to all CCS, County, State, and Federal requirements and protocols. Coordinate with County Health Services staff and State CCS consultants. Maintain confidentiality and comply with HIP nd County privacy standards. Participate in quality assurance and compliance monitoring. Minimum Requirements Bachelor's degree in Physical Therapy. Graduation from an accredited School of Physical Therapy. Current and valid California Physical Therapy license. Approved CCS-paneled provider. Preferred Qualifications Knowledge of Lake County demographics and community health needs. Experience working with children's health programs. Prior experience with State-funded or County government health programs. Experience with documentation, evaluation, and reporting in compliance with CCS standards.
