Physician - Hematology Oncology Frisco

Summary: St. Anthony Summit is looking for a locum Hematology oncologist to help with coverage gaps. All shifts are 8a-4:30p. Currently the site does not have any ongoing dates needed. BID DUE DATE: Dec 6th at 5p EST! Rate Caps for Submission: Malpractice - Must be included in rates. Orientation - half the hourly/callback ratekicks in after 8 hours. "Per hour of orientation or training beyond 8 hours" Hourly -and below - "Per hour, not OT" Holiday -Must be 1.5x the regular rates "Per holiday hour, no OT" Schedule/Availability Requirements: All shifts are 8a-4:30p Jan 21-22 Jan 28-29 Details: 8a-4:30p only - No ongoing dates are needed. Facility Location: 360 Peak One Dr, Ste 300., Frisco, CO 80443 Setting: Outpatient FTE: 0.01 Days per month: Sporadic dates of coverage as the team needs Required Procedures: Hematology/Oncology, Chemotherapy Supervision, Chemotherapy Visits, Consults/New Patient Visits, Follow ups/Established Patient Visits EMR: EPIC Support Staff: RN Reason for Coverage: Vacation coverage EMR Required Training: Module (learn lessons) - Must be completed prior to virtual EMR training to attend the training. EMR Virtual training - Only able to attend if Modules are completed before training. Virtual training must be completed prior to the providers first day. Travel Preferences: Preferred Lodging: Preferred hotels Air Travel: Air travel okay. Travel exception requests will be denied if this information is not part of the provider presentation. SUBMISSION REQUIREMENTS Board Certification Medical Oncology - Required or Truly BE (0-5 years of residency completion) Active CO license - Required or IMLC (LOQ in hand at time of submission) Active BLS - Highly Preferred Candidate must be available for any of the dates detailed in the job description to be considered - Required Must acknowledge candidate understands the EMR training requirements set forth in job description and will complete in this order to avoid missing their start date - Required Candidate must be able to treat hematology patients - Required to note Clean MP/background - Highly Preferred, site will place priority for candidates with a self inquiry NPDB at time of presentation.
