We’re seeking a compassionate and skilled In-Home Physical Therapist (PT)to provide high-quality, patient-centered care in the comfort of patients’ homes throughout Springfield, MA and surrounding areas.If you’re passionate about improving mobility, function, and quality of life for your patients — and enjoy the autonomy of working in the community — this role is for you.Key Responsibilities for Physical Therapist (PT):Conduct thorough initial evaluations and develop personalized treatment plans tailored to each patient’s needs.Provide skilled physical therapy interventions to improve strength, balance, mobility, and functional independence.Educate patients and caregivers on exercises, safety techniques, and home modifications to prevent falls or injury.Monitor patient progress and modify treatment plans as needed to achieve goals.Document all evaluations, treatments, and progress notes accurately and in compliance with agency and regulatory standards.Collaborate with physicians, nurses, and other care team members to ensure seamless continuity of care.Provide professional guidance and support to patients and their families to encourage active participation in care.Maintain equipment, supplies, and records necessary for home therapy sessions.Qualifications for Physical Therapist (PT):Licensed Physical Therapist (PT) in Massachusetts.Previous experience in home health or outpatient physical therapy preferred.Strong assessment, communication, and problem-solving skills.Ability to work independently and manage a flexible schedule.Reliable transportation and valid driver’s license.About Excel Home Care ServicesFounded in 2007 by a Registered Nurse, Excel Home Care Services has been serving Western Massachusetts with compassionate, high-quality home health care.
