(/+ $10k FYS bonus + $5k sign-on bonus) Compensation: This is a PPV position. When you apply, you will be sent the PT PPV Rates for rates may vary based on type of service and level of experience. Hours needed: 40 hours per week Must have IL PT license, 1 year of PT exp. (no new grads) 1 year of Home Health exp. Preferred but not required (can have inpatient rehab in place of Home Health) As a Physical Therapist in our client's Home Health team, you will be responsible for providing Physical Therapy services to patients in their homes, assessing patient functioning levels, and developing treatment programs to improve independence and physical/cognitive functioning. You will also be involved in patient and caregiver education, supervision, and reporting on patient progress. Key Responsibilities Perform Physical Therapy assessments and develop treatment programs Record and report patient's reaction to treatment and condition changes Instruct patients and caregivers in therapy programs and device use Supervise physical therapy assistant and, when appropriate, the certified nursing assistant Prepare reports and participate in staff and Agency development activities Schedule and Shift Details: Schedule: Monday-Friday; day shift (start & end times vary based on the patient needs) + 1 weekend every 6 weeks (Saturday & Sunday day shift w/scheduled patients) & 2 holidays per year (1 summer, 1 winter on a rotating basis); no on-call Travel
