Summary: The Hope Cancer Care Center at CommonSpirit Longmont United Hospital recently opened a new hematology/oncology clinic with a dedicated chemotherapy treatment space for patients to receive their cancer treatment. Coupled with our Breast Center, breast surgery capabilities, and our external beam treatments in radiation oncology, this role will be responsible for the treatment of hematologic diagnoses and medical oncology diagnoses in our cancer center. Physician will be responsible for clinic coverage and chemotherapy supervision 4 days per week. As a new clinic, this role is also responsible for building/growing this subspecialty practice. BID DUE DATE: Dec 6th at 5p EST! Rate Caps for Submission: Malpractice - Must be included in rates. Orientation - half the hourly/callback ratekicks in after 8 hours. "Per hour of orientation or training beyond 8 hours" Hourly -and below - "Per hour, not OT" Holiday - Must be 1.5x the hourly rate"Per holiday hour, no OT" Schedule/Availability Requirements: Jan 6, 2025-Dec 31, 2025 Details: One full time provider to fill the need. Monday-Thursday, 7:00-5:00 PM with 30 min unpaid lunch. EMR: Epic Support Staff: RN Reason for Coverage: New Clinic - Currently Recruiting Permanent Physician EMR Required Training: Module (learn lessons) - Must be completed prior to virtual EMR training to attend the training. EMR Virtual training - Only able to attend if Modules are completed before training. Virtual training must be completed prior to the providers first day. Travel Preferences: Preferred Lodging: Preferred hotels Air Travel: Air travel okay Please detail any anticipated travel exceptions, for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation. Please also indicate the city where provider will be traveling from in the travel section of the provider submission. Credentialing Information: Timeframe: 60 days temps / 120-180 days Full privs Emergency privileges? Yes Emergency within 7 business days with a clean file only. Credentialing with other facilities? Currently about 4 clinic patients per day. Additional Info: Boulder County SUBMISSION REQUIREMENTS Board Certified Medical Oncology - Required Active CO license - Highly Preferred, if candidate presents with IMLC (LOQ must be in hand at time of submission) Active BLS - Required for credentialing Active DEA - Required for credentialing (site will consider a transferred DEA, however, must keep this DEA for the duration of the assignment, not just for shifts worked.) Candidate must be comfortable and willing to see Hematology patients - Required - Must note at name clear Must acknowledge candidate understands the EMR training requirements set forth in job description and will complete in this order to avoid missing their start date - Required Must be available to commit to a 1 year commitment at minimum - Required Candidates must stated that they are aware that this role is responsible for building and growing this subspecialty practice - Required to note at name clear. Candidate must be available Mon-Thurs, 7a-5p and Fridays 2 hour remote Triage - Required to note Clean MP/background - Highly Preferred, site will place priority for candidates with a self inquiry NPDB at time of presentation.
