Job Title: IHA Per Diem Nurse Practitioner / Physician Assistant Reports to: Manager, Care Delivery FLSA Status: Exempt Update: Candidates must have PCP experience or in-home health experience. Preference is given to those with both. Monthly Requirement: Minimum of 30 completed visits. Travel: Up to a 50-mile radius. Position Summary The Provider performs in-person, no-cost annual preventive Health Risk Assessments (HRAs) for Medicare, Medicaid, and Affordable Care Act members across designated health plans. Assessments occur within the member's home and must meet all market and health-plan expectations. Schedule Flexible daytime and evening availability Appointment durations: 60-75 minutes Compensation Market-rate pay per completed assessment Mileage reimbursed after the first 30 miles driven each day at the current IRS rate Essential Functions Providers must be able to meet the following expectations: Complete all required self-paced and instructor-led training within designated timelines. Provide a minimum of 30 appointment slots per month. Deliver high-quality assessments that meet member satisfaction, health-plan requirements, and internal quality standards. Submit chart notes within 24 hours of each visit. Respond to charting queries within 48 hours. Maintain strong communication with clinical leadership. Participate in periodic training sessions and role-related meetings. Comfortably enter and work within members' homes. Lift up to 20 lbs as needed. Bend or kneel for extended periods when required during assessments. Position Requirements Core Competencies Strong computer proficiency and experience with electronic medical records (EMRs). Understanding of CMS and ACA risk-adjustment models. Ability to document findings accurately and thoroughly. Effective verbal and written communication skills. Ability to maintain confidentiality and comply with HIPAA regulations. Sound decision-making skills aligned with policies and procedures. Adaptability in a dynamic work environment. Positive, collaborative approach with members and clinical teams. Ability to proactively identify and resolve member, provider, and health-plan issues. Education & Experience Master's or Doctorate degree from an accredited Nurse Practitioner program (FNP, AGNP, ANP). OR Master's degree from an accredited Physician Assistant program. Home health experience within the last 5 years (Required). At least 1 year of in-home assessment experience (Highly Preferred). Certification & Licensure Active RN and NP or PA license in the state of practice. Board certification through ANCC, AANP, or NCCPA. Active state Medicaid ID. Active Medicare PTAN. Current BLS certification. Valid, unencumbered state driver's license. Proof of adequate auto insurance for the state in which visits occur.
