Responsibilities: Assess the psychosocial needs of patients and families to develop individualized care plans. Provide counseling and emotional support to patients and caregivers. Assist in connecting patients and families to community resources, financial assistance, and legal aid. Collaborate with nurses, therapists, and physicians to ensure comprehensive care planning. Document all services provided in compliance with agency policies and regulatory requirements.
