Job Description

This position is for the after-hours coverage for hospice patient needs in the hospice service areas of Allen, Steuben, DeKalb, Noble, LaGrange, Whitley, Huntington and Wabash Counties. After-hours coverage needs will be shared with other nurses on the after-hours team as well as 12-hour shift visit nurses. The after-hours Hospice team plays a vital role in supporting patients and families with needs that arise after 5P. Visits provided for Hospice patients can include admission visits, PRN visits for symptom control and patient decline, medication adjustments and coordinating transfer in setting needs as they arise. This position blends clinical expertise with compassionate communication and seamless coordination to ensure timely, smooth, and patient-centered admissions and visits within our area hospitals, skilled nursing facilities, or home settings. Must have valid state driver's license, reliable transportation, and proof of auto insurance. Must have current CPR certification.Other QualificationsMust have good verbal and written communication skills. Must be able to lift 50 pounds, assist with 100 pounds. Demonstrates working knowledge of computer systems. Must demonstrate competency in the delivery of patient care including competency based practice and age specific care.
