Home Health Aide Erie County

UPMC Home Healthcare and Family Hospice is seeking a compassionate and dependable Home Health Aide to provide exceptional, one-on-one care to home health and hospice patients throughout Erie County. This field-based role offers the opportunity to make a meaningful impact by supporting patients in the comfort and dignity of their own homes. This position is a full-time, Monday-Friday position from 8:00AM-4:30PM with rotating weekend shifts. Responsibilities include: Providing safe, direct personal care under the supervision of licensed professionals Monitoring patients' conditions and promptly reporting changes to the interdisciplinary care team Supporting patient comfort, well-being, and overall satisfaction Completing accurate and timely documentation in accordance with agency and regulatory standards Collaborating effectively with team members to ensure coordinated, high-quality care Participating in required meetings, training, and annual competency evaluations Adhering to organizational policies, standards of conduct, and compliance requirements The ideal candidate demonstrates strong communication skills, professionalism, reliability, and the ability to work independently in a home-based setting. Qualifications: High School Diploma or GED - Required One (1) year of experience - Preferred Certified Nurse Aide - Preferred Minimum of 75 hours of Nurse Aide training - Required. If no previous nurse aide training, applicant may attend UPMC Home Healthcare Nurse Aide Training Course as available upon hire.
