Case Manager / LTSS Assessor (RN) – Field-Based (Onondaga County to cover Onondaga and surrounding counties of Oneida, Madison and Cortland) / Senior Field RN Care Coordinator – UAS Assessments Location: Syracuse, NY (Field-based)Compensation: $50 – $58 per hour + mileage reimbursementBonus: Up to 8% Annual IncentiveEmployment Type: Full-time We’re currently seeking a Senior LTSS Assessor (RN) for a field-based role focused on conducting UAS assessments in Onondaga County to cover Onondaga and surrounding counties of Oneida, Madison and Cortland. This position plays a key role in coordinating long-term care services for members with complex medical needs. Position Purpose: As a Senior LTSS Assessor, you will: Perform comprehensive UAS assessments and reassessments in members' homes or other sites (2–4 daily)Develop, implement, and monitor individualized long-term care plansCoordinate medical and support services across the care continuumEducate members, families, and caregivers on care options and resourcesCollaborate with internal and external care teams to ensure quality, cost-effective, person-centered carePrecept and mentor new clinical hires, contributing to high-quality care and staff development Qualifications: Must-Haves: Active Registered Nurse (RN) License in New YorkResidency in Onondaga County to cover Onondaga and surrounding counties of Oneida, Madison and CortlandMinimum 4 years of relevant RN experienceExperience conducting UAS assessmentsValid Driver’s LicensePersonal vehicle preferred, but public transportation will be consideredComfortable being in the field Monday–Friday Preferred Skills & Experience: Bachelor’s degree in Nursing (BSN)Experience in care management, ideally within a Managed Care Organization (MCO)Familiarity with state and federal regulationsStrong understanding of community resources and care coordinationAbility to mentor and support clinical staffExperience with high acuity/complex populations Education & Experience: Graduate from an Accredited School of Nursing requiredBachelor’s degree in Nursing (BSN) preferred4–6 years of nursing experience, ideally in care management or similar settings What We Offer: Competitive hourly rate: $50–$65/hour (based on experience & qualifications)Annual bonus incentive up to 8%Comprehensive benefits package, including:Medical, dental, and vision coverage401(k) with company match & stock purchase plansTuition reimbursementPaid time off and holidaysFlexibility with field, remote, and office-based work options Katherine Skelton 586-710-0022
