Position Summary The Hospice Registered Nurse provides, coordinates, and directs compassionate end-of-life nursing care to patients in their homes or other care settings. Care is delivered in accordance with physician orders, agency policies, and established hospice principles. The RN supports patients and families through the physical, emotional, and spiritual challenges associated with terminal illness. The Hospice RN reports to the Clinical Manager and supervises Home Health Aides and Licensed Practical/Vocational Nurses (LPNs/LVNs). Key Responsibilities Conduct comprehensive assessments of patients' physical, emotional, psychosocial, and spiritual status. Develop individualized hospice plans of care in collaboration with the interdisciplinary team, patients, and families. Provide skilled nursing care focused on pain management, symptom control, and comfort measures. Monitor and evaluate the effectiveness of interventions and adjust care plans accordingly. Communicate significant changes in patient condition promptly to the physician and care team. Educate and support patients, families, and caregivers on disease progression, symptom management, medication administration, and end-of-life care. Provide emotional support and guidance to patients and families, including grief and bereavement resources. Supervise and direct Home Health Aides and LPNs/LVNs in patient care activities. Complete accurate, timely clinical documentation and progress notes. Participate in interdisciplinary team meetings and case conferences. Maintain compliance with agency policies, professional standards, and applicable regulations. Travel & Work Requirements Regular home or facility visits within the assigned service area. Periodic in-office meetings and trainings. Reliable transportation, a valid driver's license, and agency-required automobile liability insurance are required. Qualifications Current and unencumbered Registered Nurse license in the state of practice (or multi-state license under the Nurse Licensure Compact, if applicable). Minimum of 1-2 years of nursing experience, with at least 1 year in hospice, palliative care, or oncology preferred. Current CPR certification. Strong assessment, critical thinking, and symptom management skills. Comfort with providing care in emotionally challenging situations. Competency with electronic medical record systems, email, and online training tools. Skills & Competencies Ability to develop, implement, and revise individualized plans of care. Advanced communication skills to effectively educate and support patients and families. Emotional resilience and compassion in caring for terminally ill patients. Excellent time management and organizational skills to manage multiple patients. Professional demeanor, ethical conduct, and respect for cultural and individual differences. Physical & Environmental Requirements Ability to safely lift, reposition, and transfer patients as needed. Work may involve exposure to infectious diseases and conditions typical of home or facility environments. Frequent driving and entering patients' homes. Pay Rate Range: $2,100 - $2,300/week Pay Rate is dependent on seniority and other factors that will be discussed during the hiring process
