Job Description

Position Summary The Family Nurse Practitioner (FNP) provides comprehensive, patient-centered primary care across the lifespan-from pediatrics to geriatrics. The FNP is responsible for assessing, diagnosing, and managing a variety of acute and chronic health conditions, promoting preventive health measures, and ensuring continuity of care. The ideal candidate will demonstrate strong clinical judgment, excellent communication skills, and a commitment to providing high-quality, compassionate healthcare. Key Responsibilities Conduct thorough health assessments, physical examinations, and diagnostic evaluations. Diagnose and manage acute and chronic illnesses in patients of all ages. Develop individualized treatment plans and prescribe medications and therapies as appropriate. Order, interpret, and evaluate diagnostic tests such as lab work and imaging. Provide health education, disease prevention counseling, and wellness support to patients and families. Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated patient care. Maintain accurate, timely, and compliant electronic medical records (EMR). Perform routine procedures such as wound care, immunizations, and minor office-based treatments. Participate in quality improvement initiatives and follow evidence-based clinical guidelines. Uphold all legal, ethical, and professional standards of care and confidentiality. Qualifications Education: Master's or Doctorate degree in Nursing from an accredited program. Licensure: Current Registered Nurse (RN) and Advanced Practice Registered Nurse (APRN) license in IL. Certification: Board Certified as a Family Nurse Practitioner (FNP-C or FNP-BC). Experience: Minimum of 1-3 years of clinical experience in family practice, internal medicine, or primary care preferred. Skills: Strong diagnostic and clinical decision-making abilities. Excellent interpersonal and communication skills. Proficient in EMR documentation and telehealth platforms. Ability to work independently and collaboratively within a team. Work Environment Primary care setting. May involve occasional evening or weekend shifts based on patient needs.
